Stay clear of Dr.Thaddeus Zawaslak. I was informed by my dentist at Community Health Net that I needed a root canal. I found that Perfect Smile could do root canals. Dr. Thaddeus was assigned to me. I informed him of the recommendation from CHN. He told me I did not need a root canal at that time. He saw no need again, six months later. Long story short, his incorrect diagnosis has ended up causing me to lose that tooth, IMO. Not only that, he repeatedly dropped instruments and needed help from his assistant simply placing the permanent crown. Not sure what his problem is. He is a very nice man who has practiced dentistry in excess of thirty five years and has, I'm certain, helped many, many people over the years!!! It brings me no pleasure, at all, having to post this message. It is something I do not take lightly, but I would be remiss not to.

Horrible office front staff is rude. They wanted to pull at least 6 teeth without sedation kept telling me that my insurance doesn't cover it. No my secondary does nobody every told me that he doesn't take my secondary until I called my insurance and relayed the info to them. Then they tell me that that doctor doesn't accept the insurance.then try getting your records from them. Ha ha ha they don't cAre that your face is totally swollen. Can't drive can't see can't do anything but lay in pain. And they don't care. I should've known better

HORRIBLE STAFF!! They treated me beyond words of disgust!! The associates treat the dentist horrible, telling him what he can and can’t do. They refused to come assist him as he worked on my tooth! They never sent my son’s paper work over to French creek, for a root canal!! When I asked for HR’s number they gave me one of their cell phone numbers! I called and she answered and then hung up. I reported this to the highest person I could reach and it’s been almost 2 weeks and I haven’t heard anything. The staff are 3 African-American women who truly treated me like I was below them!!! This all started over a month ago because I canceled an appointment!!! They need their jobs taken! And this place needs shut down!!!!

OK, so I went to this dentist today to get some fillings done. He came in numb my mouth and then left her about a half an hour which is fine. It’s understandable, but then I started looking at the instruments. They used like look inside my mouth like you know the thing that pokes your teeth and the mirror and everything looked really dirty and used like it was for the person before me And so I asked the lady I said can I get a new set of utensils for cleaning so yeah I don’t know. Sometimes that happens when we disinfect the utensils. It will be like spotting and stuff like that. There isn’t just spotting on the other tools. There was like white stuff from the last person so she takes it all. She comes back with a sealed bag of utensils so wait I’m wondering if you’re just disinfecting all your utensils why did you come back with a sealed bag of utensils?
