My husband and I went in to get his physical for the county since we are becoming foster parents. Prior to coming to the appointment I had called and made sure the paperwork wouldn’t be an issue and they informed me it was fine but “outside” paperwork is $45 to be filled out. Before heading back to a room he was charged the $45. Once we went back to a room is when the paperwork became an issue, AFTER we had already paid. I’d like to reiterate that this form was for a physical exam. Not a psychological exam, not bloodwork, a very simple form simply asking if this person is in good health to become a foster parent. The nurse or assistant lets us know that the doctor will not fill the form out unless we do lab work and get a note certifying that my husband’s “condition” is “well controlled” from his psychiatrist. My husband has adhd lol. Of course this is frustrating when this office is 40 minutes away from our home, we’ve already paid, and I had talked to front desk before who assured me getting the paperwork done wasn’t an issue. Now, normally, for these types of forms, neither bloodwork nor a note from a psychiatrist are required. And I in fact, did my physical fairly quickly and did not have to do these things. HOWEVER, I understand this office has different protocols but the actual doctor left for the day and didn’t even come into to speak with us or talk to us. These were his instructions relayed by his staff yet he never once came into the room despite my husband being his patient. It truly felt like he saw “adhd” on his chart and immediately discriminated us because he didn’t even come in during this appointment at all. Our frustrations aside, the most disrespectful part of this visit wasn’t even the time wasted. It was the manner in which both members of the staff spoke to my husband in regards to his “condition.” Stating things like “well we don’t know you, how could we fill this out?” & “this deals with children, we need to make sure we aren’t held liable if his condition isn’t under control.” Mind you, AGAIN, his “condition” is adhd which he has had the entirety of his time at this doctor’s office. To insinuate that adhd would cause someone to put a child in harm or danger is an outrageous and disrespectful claim. All while they didn’t even perform an actual physical. And of course the staff doesn’t know him? Why would they unless he comes in routinely because he’s not healthy. Not once on this form does it state you need to know the patient for a certain amount of time, it does not mention bloodwork, and it does not mention a psychiatric evaluation but again, regardless of their different protocols, the way in which they treated controlled adhd as a debilitating mental condition was beyond rude, disrespectful, & wrong so of course we were absolutely frustrated. Upon leaving, we were refunded our $45 and the gal at the front desk was even shocked that we didn’t get our form filled out. The other staff member interrupted her and essentially told her not to speak to us. While I understand that my husband and I were visibly upset, it does not help when staff can’t even acknowledge the way we feel and continue to say they need to be careful on their end in case us fostering goes poorly. We ended up walking into a CVS and getting an actual physical on the spot and had our form filled out no problem and up to standard. This only confirmed that we were being singled out for either profit through more referrals or we were simply being singled out because the doctor and staff there see adhd as a terrible and risky condition to live with. I highly recommend that if you need a physical form, head to CVS minute clinic or any other doctors office where they won’t treat you like your insane for having something very normal.

DON'T WASTE YOUR TIME I called 15 times over 2 days to get to speak to someone to schedule an appointment, I left numerous messages but no one called me back. When i managed to get to speak to someone the next appointment was 10 days away so I booked it, I told them my insurance and all was scheduled. 3 days before the appointment I received an email saying it was canceled. I tried to call to confirm this and after 7 attempts I finally spoke with someone and they said the doctor has taken the day off so we canceled your appointment. The next day I receive a text message confirming my appointment? I was very confused as they said it was canceled, I tried calling throughout the rest of the day and after over 20 times I managed to speak to someone and they said they was unsure of what was going on and someone will call me back ASAP. But no one called me back. The next day after 5 attempts of me calling I spoke with someone and they explained they canceled my appointment at Summerlin but moved it to MLK location. It would of been nice to of been informed of this but I was just happy to have the appointment. I went to my appointment to check in and handed over my ID, after a few minutes the lady told me they dont except my insurance. I asked is this the same as Summerlin because I was told they except it. She said its the same and not excepted at either practise. I left and tried to call to see if there was anyone higher up I could confirm this with. The lady that I spoke with at reception answered and I asked if there is a number of a corporate office or anyone I can speak with and she gave me a number. I called the number and she answered, she gave me their phone number. She said she can get a manager to speak with me, and I was put on hold. She came back and said the manager was busy and the manager will call me back. I asked if this can please be done as I have left numerous messages before and been told someone will call me back and no one ever has. She said if I want an appointment I can pay $149 , I replied with I wouldn't be able to pay $10 for an appointment as no one answers or calls me back. She replied with I'm hanging up on you now. And hung up. And im still waiting for the manager to call me back

Had the best experience with faculty anyone could ever hope for Yully and her staff were amazing they all went above and beyond to help me be able to take my drug screening I recommend this place
